The autumn issue of Dermatology in practice challenges us to think about the way dermatological services are provided for our population. As usual, Barry Monk leads the charge, in his inimitable way, reflecting on the whole process from GP referral and onwards through the hospital outpatient systems. He explains the value of the specialist outpatient departments and the benefits of super-specialist clinics and management (while later promoting the value of the general dermatologist in his Monk’s Moment). Tim Cunliffe expertly describes the changes in accreditation for GPs with Extended Roles in the clearest description I have read to date.
